Plan and Do is how Plumtree partners with families of children aged birth to five years to thrive. First we plan, and then we do.
What happens in Plan and Do?
- Practical sessions with your therapist or early childhood specialist to help you work towards your goals · Partnering with a key worker as your main contact if you have more than one professionals working with you.
- Planning with your key worker or therapist to develop a clear plan, so everyone knows what to aim for
- Regular conversations and check-ins to keep your plan on track and celebrate outcomes
- Support to help you search for information and make decisions
Keeping track of progress
Plan and Do has three parts, each with a different focus.
- We get started by working on planning and trying out new ideas
- We get going through practical sessions, with time to evaluate and plan for the coming week
- We plan next steps and discuss what happens when your block of sessions finishes or when your NDIS plan is nearing the renew date
For children who are in childcare or preschool
Your key worker can visit your child at school or in childcare and work closely with the staff on the goals in your plan. We will meet with you regularly to check progress and make any necessary changes to suit your child’s and family’s needs.
